Services Overview

This is an initial psychiatric evaluation with a prospective patient. The first visit is 2 hours long. During this visit, I will perform a formal psychiatric evaluation including clinical history, mental status exam, review of relevant documentation. For patients under 18 years old, I will also obtain information from parents and guardian.  If your first appointment is through telehealth, please bring any recent visit notes from your primary care providers including vitals, physical exam, weight, and labs.  A provisional diagnosis is usually established on the first appointment. Evaluations and more definitive diagnosis may require multiple sessions. If you decide to continue treatment with me, we will create an individualised treatment plan. If clinically appropriate, we may start treatment on the first visit. Please sign up for the Luminello Patient Portal which will have intake forms and consents that need to be completed prior to visit.

This is a medication management visit that lasts between 15 to 25 minutes. This visit is best suited for patients who already have an existing therapist. Consistent therapy with a clinician with whom you have a good working relationship is the strong foundation on which good psychiatric care is built.

These visits are suitable for patients on medications who do not have a primary therapist. If I prescribe you or your family member medications, I also offer therapy because integrated treatment is oftentimes better than medication alone. My sessions are dynamically focused with an emphasis on building insight. When appropriate, I also utilise cognitive behavioural therapy (CBT) and supportive therapy. 

This is a collaborative model with an emphasis on working with the primary care treatment team such as your family doctor or pediatrician. This visit is usually a 2 hour visit in which I see you in a consultative capacity. The goal is for you to return to care to your primary care provider with recommendations from a specialist. I will coordinate care with your primary care provider. One advantage of this model is that it can be more affordable for you or your family.  Session includes a comprehensive diagnostic evaluation and care coordination with the referring doctor. Certain cases may require additional evaluations for diagnostic clarity or treatment monitoring.

I do not charge for brief telephone discussions. I strongly recommend extended clinical discussions be done during a formal appointment. Extended phone calls or repeated phone calls in absence of a follow up appointment are subject to fees. Please note that I do not use email or messaging for extended clinical discussion. 

In addition, I do not charge for brief forms/letters such as school or camp medication forms, FMLA paperwork, or prior authorization requests. More extensive documents such as those required for insurance appeals or IEP/504 services will be subject to charge.
